What? Are you accusing me of questioning you? What kind of proposition is that? Where would you ever get such an idea?

Can you tell me how to get that event to appear?

Well Cao Xing did hit Xiahou Dun in the eye, so Koei probably put that event in to maintain chronological sense for Xiahou Dun.

You don't need Cao Xing to attack Xiahou Dun in order to get that to happen.
I was once in the Rise of Heroes scenario when a random soldier told me that
Xiahou Dun lost his eye in battle. (Xiahou Dun wasn't even in a battle at that time)

This is accurate. I was just performing domestic tasks and got an event where a soldier informed me that Xiahou Dun was injured while training and lost his eye.

He was definitely sitting somewhere doing nothing at the time. He has to lose it somehow, right?

yep. but when in battle, after he loses his eye, his unit got a will boost

I find it funny when you send an annual envoy to your close Ally:

Ally: Welcome ____'s Army, what can I do for you today? =D
Envoy: I regret to inform you this, but we'd like to dissolve our alliance as of today D:
Ally: *stands up and hair sets on fire* Then we'll be seeing each other on the battlefield before long! ...Now, can you kindly escort the envoy outside and show them the way...

Also

Envoy: We'd like to release _____ for this amount of gold (1k) [minimum price needed usually in this case for player: 1.5-3k]
Player: [decline] We can't be expected to agree to these terms, perhaps if you had more to offer..
Envoy: I see, I'm sure you'll live to regret this some day! Now..where's the exit, kind sir.

(Not 100% accurate, but still..nobody pulls a Yuan Shao or -insert enraged and merciless warlord here-)

It's both.

The bolded part isn't true according to the game, but it's very realistic. You don't just say a snarky retort or comment during a diplomacy and expect to find your way or be guided out safe and sound.
